If cell A = value and cell B equals a different value then copy cell C from another sheet

Okay so i'm having a very difficult time to get through this and you guys are my last resort. Let me try to explain what I'm doing.

I have a sheet filled with information from a google form. Now this form is used by different people and they differ themselves by putting in what they do in the company.

Now what I need is an overview from one particular side of the company. So what I want is to have a different google sheet that pulls information from the first sheet.

If cell A matches the number that is also in cell A in the new sheet, and cell B (in the old sheet) matches the words PPXL then I it to copy what's in cells C and D (from the old sheet).

I hope it's clear by what I just wrote above, if it isn't, i'll explain it more indepth with clear pictures of what information I have and how it needs to be copied.
