Google form that sends conditional responses to conditional email address

I have a google form that needs to send responses to different department emails based on the request type. The required input responses vary depending upon the request type. I am a very amateur coder but if you geniuses can give me the script for the following example, I should be able to resolve the rest on my own.

If the request: A new membership(in dropdown menu), send the input data in "Name","Birth Date" & "mailing address" boxes to global@fitness.com

If the request: Renew Membership (in drop-down menu), and the input data in "City" = "San Diego" send the input data in "Membership No." ,"expiry date" & "transit" to sandi@fitness.com whereas,

if the request: Renew Membership, and the input data in "City" = "Los Angeles" send the input data in "Membership No." ,"expiry date" & "transit" to la@fitness.com

Please help a bro out on this.
