If else statement for radio buttons in form

I have a request a quote form under construction and i'd like to use if statements for the radio buttons in the form. http://ift.tt/2rPkiyQ

Basically what I'm trying to accomplish is for each radio button (step 3 in the form after What's your name and email address) to take you to a different question in the form and after displaying that question, return to the normal flow of the form "What's the primary color for your website" "What's your budget" etc. I've been trying to get this to work but what i'm trying isn't working unfortunately. I'll appreciate if anyone can help me!
