Combining text strings in Excel based on conditions using formula or VBA

I have Column A filled with postcodes/zip and Column B filled with a separate list of postcodes/zip. Column C & D both have category text.

Example

If there is a postcode in both Column A & B then I want Column D text to be added on to Column C text. How do I do this ?
