Condition after an item of radio button is checked

Suppose I have web form(asp. Net - c#), and I have to fill some fields, then there is a radio button in which if I check one of its items, the next fields should be filled directly by the same content I alrady filled. For more clarification, suppose I fill my personal data (name-age-gender..) then there is a radiobutton to choose whether I want to apply the form for myself or for some one else, and if i choose the option of myself, the next fields needed to be filled directly with my personal data that I already filled.

Please guide me how this can be solved?
